come out 是不及物动词，没有被动语态。 只能是sth come out，不能是come out sth，所以不能是sth is come out。

Once a month 是一个月出一次的意思 所以要用 How often come out 要用do 做助词
杂志是主语 问题是杂志多久出版一次 不需要用被动

这里come out是不需要被动的、、

根据题目的第二句话"Once a month"（一个月一次）可知道提问的是频率，而不是问具体的时间，因此用How often而不用When，排除C、D。
日常动作用一般现在时，the magazine （杂志）是第三人称单数，作主语。杂志出版、发行，不是被动语态。
the magazine comes out once a month .(这杂志一个月出版一次)

往年高考题中有这样一句话：The dictionary is being printed and it will soon come out. 就没用被动。
